A Bilateral Cleft Lip and Palate Patient Treated by Maxillary Advancement: A Case Report

Abstract
The purpose of this study is to present improvement of facial features in bilateral cleft lip and palate patients through Le Fort I maxillary advancement. The patient was a 17-year-old bilateral cleft lip and palate patient with a chief complaint of upper lip depression and anterior crossbite. The patient and his parent strongly wanted non-surgical orthodontic treatment; hence, therapeutic diagnosis was tried by adjusting maxillary anterior teeth inclination and restoring maxillary lateral incisors for improving overjet. The patient asked for additional improvement of facial appearance. Therefore, Le Fort 1 maxillary advancement was applied to move the maxilla forward and downward. ANB was changed from ?3.3 degrees to ?4.1 degrees during decompensation, became 2.6 degrees right after the surgery, and ended in 0.3 degree when the treatment was over. However, FH-U1 was increased from 101.3 degrees before the surgery, to 111.6 degrees right after the surgery, and to 118 degrees at the end of the treatment. Intermaxillary relationship and facial features were highly improved. In this case, therapeutic diagnostic procedure and treatment plan adjustment were parts of continuous communication with bilateral cleft lip and palate patients who were skeptical regarding his surgical procedure. Finally, orthodontic treatment with Le Fort 1 maxillary advancement was fulfilled, resulting esthetic and functional treatment outcome.
